- comments: observed approximately 30 mouse droppings on the floor of the rear storage room. Management instructed to clean and remove all rodent droppings from the affected areas. Additional pest control service is needed to eliminate the pest activity. Priority foundation violation 7-38-020(a). Citation issued.
53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed the 2nd floor employee washrooms without a self-closing door device. Management instructed to install and maintain.
55. Physical facilities installed, maintained & clean
- comments: repair the bottom of the corner wall at the entrance to the pizza station. Replace the missing wall tiles and seal the holes in the walls of the 2nd floor prep area.
58. Allergen training as required
- comments: observed the certified food manager without an allergen training certificate. Management instructed that all certified food managers must show proof of training.
