- comments: observed an infestation of appx. 75 or more live roaches crawling in between wall bases and gap in wall next to biscuit prep table,inside of electrical conduit on floor underneath prep table & along both sides of conduit leading in between equipment and in crevices of under side of prep equipment,mixed in with trash ,food debris and bags on floor under same. Also appx. 30 or more mice droppings on shelving units and outlet box at drive-thru. Must remove droppings clean and sanitize areas. Must have a pest control operator to service premises on or after 1/5/24. (priority 7-38-020(a)(citation issued)

Must repair and maintain. #49- 4-602. 13 : observed spilled food residue on the inside and outside of thawing cooler, cooking equipment,storage shelving, prep stations and hot holding unit in the prep area. Instructed to detail clean and maintain to prevent possible food source for pest. #55-6-501. 11 : observed excessive spilled food debris and grease accumulation under cooking equipment and the floors throughout the facility. Must detail clean and maintain to prevent possible food source for pests. #57- 2-102. 13 : observed food handlers without food handler certifications.
