    My husband and I went to Anna Maria Oyster Bar in Ellenton Florida on Sunday March 10th at 3:30 p.m. I ordered a hamburger medium well done. It was served well done, but I still ate it.
    About 4 hours later I started having serious diarrhea and... See More vomiting with stomach cramps and trembling. I experienced extreme fever and chills throughout the night. I spent the whole night getting up to get on the toilet and hold a trash can, to vomit in.
    All day Monday March 11th I continued to have serious diarrhea and vomiting and severe weakness and chills. I could only get out of bed to get on the toilet and vomit over and over. My husband got electrolytes and anti- diarrhea pills to help. The diarrhea and vomiting continued throughout the day and night.
    On Tuesday March 12th I continued to have serious diarrhea throughout the day and evening and overnight.
    On Wednesday March 13th I decided to go to the Manatee Memorial Hospital emergency room in Bradenton Florida. Because the Diarrhea and nausea continued. After 5 hours at the hospital, I was diagnosed with salmonella, I was put on a IV fluid drip bag to rehydrate me and to stop the diarrhea and nausea for an hour and a half and given antibiotics and cramp and nausea pills. Even after being released from the hospital, I continued to have diarrhea and cramps throughout the day and all night.

    - comments: observed no city of chicago certified food manager on site during the time of inspection with a valid original certificate while preparing and serving pizza, hamburgers, sauce, and beef.     - comments: no written and signed employee health policies on site for review for all food and conditional employees. Instructed to provide and maintain on-site. Priority foundation violation. 7-38-010. Citation issued.

    - comments: observed no hot running water at handsins located in front prep or (in use) pizza prep at this time. Noted front prep handsink 64 f and pizza prep without any running water during inspection. Instructed to provide hot running water at minimum required temperature of 100f and maintain. Priority violation. 7-38-030 (c). Citation issued.

    33. Proper cooling methods used; adequate equipment for temperature control
    - comments: observed 3-door cooling unit in pizza prep area not at required cooling temperature. Noted temperature @51f. Instructed not to use, repair, and maintain temperature of 41f or below at all times. Priority violation. 7-38-005. Citation issued. Unit tagged and held for inspection, no tcs foods are stored inside at this time. Instructed to use upright pepsi cooler located in prep to store foods.

    - comments: observed approximately 170 large rodent droppings on premise in the following areas. . . Front prep area on floors under heavy equipment, in pizza prep in corners behind 3 door cold hold unit, rear prep/dishwashing area scattered under grease trap and unused prep sinks by bulk seasonings, alongside and inside pizza oven, in rear dock/dry storage under all shelving units on ground and along garage door. Instructed to contact pest control for service, remove droppings, clean and sanitize all said areas. Instructed to eliminate pest activity. Priority foundation violation. #7-38-020 (a)citation issued.

    - comments: observed front entrance, back door and garage door with at least 1/4 inch gap between the floor and door. Instructed torodent proof said doors in order to prevent pest entry.

    - comments: observed all heavy equipment throughout front and pizza prep areas with accumulated food debris and splatter. Instructed to clean and maintain.

    55. Physical facilities installed, maintained & clean
    - comments: observed missing and water-stained ceiling tiles in ceilung above pizza prep cold hold unit. Instructed to repair and replace all attachments.

    56. Adequate ventilation & lighting; designated areas used
    - comments: observed ventilation hood in front prep area with accumulated dust, dirt an grease built up. Instructed to clean and maintain.

    - comments: observed no food handler certificates on site for review. Instructed to provide.
