    Rice is a common cause of food poisoning. Uncooked rice can contain the food poisoning bacteria Bacillus cereus. The bacteria survive even after the rice is cooked, and mishandling can lead to toxin production. Onset of symptoms can be as soon as an hour after and include vomiting and diarrhea.

    - comments: observed no hand washing soap available at the hand washing sink in rear dishwasing area. Instructed to provide hand soap at all hand washing sinks for proper hand washing.     - comments: observed hand washing sink in front prep area not accessible for use. Sink blocked by a large garbage can, soiled wiping cloths and knives being stored inside the sink. Instructed person in charge that hand washing sinks must be accessible at all times and are for hand washing purpose only. Priority foundation violation 7-38-030(c). Citation issued.

    - comments: observed no handwashing signage posted near hand washing sink in the unisex employee/customer washroom. Must provide and maintain.

    - comments: observed hot time temperature control for safety food at improper hot holding temperatures. Observed a big container of cooked rice with temperature of 114. 5f in front prep hot line area. Instructed to maintain all hot foods at 135. 0f or above at all times. Management immediately and voluntarily discarded approximately 10lbs of food value of $80. Priority violation 7-38-005 citation issued.

    - comments: observed cold time and temperature control food safety food at improper cold holding temperatures. Observed bucket of seasoning yogurt raita stored at room temperatures next to walk-in cooler with temperature of 57. 7f. Nstructed to maintain all cold food temperatures at 41. 1f or below at all times. Manager immediately and voluntarily discarded food approximately 40 lbs value of $70. Priority violation 7-38-005. Consolidated with above violation.

    - comments: observed evidence of pest activity. Observed approximately 35-40 mice droppings on site. The following areas were observed with mice droppings on the floors: under the three compartment sink in the main food prep area surrounding the dishwashing chemicals, in the rear food storage area behind the boxes of vegetable oil. On the floors in the prep area in the basement in the following areas: near the pipes above the grease trap next to the three compartment sink, in the corner near the metal shelving unit across from the dry ingredient containers, in the mop sink closet, on the side of the walk-in cooler, under the black shelving units across from the walk-in freezer, surrounding the equipment in the office, in the alcohol storage closet, surrounding the orange bins stored against the wall near the stairs leading upstairs to the bar. Instructed to eliminate evidence of pest activity, clean and sanitize affected areas and recommended contacting pest control for a service. Priority foundation violation 7-38-020(a), citation issued
