    - comments: observed 4 boxes and packages of frozen tcs foods such as: chicken wings, pork tips, pork tails, package fish sitting in at room temperature in the shopping aisles near the meat/seafood prep area. Instructed manager... See More that leaving frozen foods in ambient air temperature is not applicable. Manager instructed employees to out all product into cold holding units.

    - comments: observed no probe stem thermometer for taking food temperatures. Must provide. Priority foundation 7-38-005, citation issued

    38. Insects, rodents, & animals not present
    - comments: observed pest infestation. Approximately over 1,000 mice droppings throughout the facility. The following areas were observed with mice droppings on the floor:in the inside of the walk-in cooler with produce stored inside, front service area by the entrance, in the box disposal/ packaging area, surrounding the aquariums with live fish across the meat/seafood prep area, under the food prep tables in the meat/seafood prep area, in the mop sink area, outside the toilet rooms, near the green picking area in the shopping area, the electrical closet at the front, in the office, behind cold holding units in the shopping aisle, and under the pallets in the basement, behind the front service counter, and in the shopping aisles espcially where the pummeto, fruits, and vegetables are stored. The following surfaces were observed with mice droppings: on the pallets in the basement, on the shelving units in the shopping aisle especially where the dried mushrooms are stored, on the slicer and the table where the slicer is stored in the meat/sea food prep area, on the ledge of the wall near the green picking area, and on the boxes in the storage area where the portable gas stove is stored, the lower interior of the cold holding unit near the toilet rooms, in the shelving units behind the front service counter. Instructed to eliminate pest activity, clean, sanitize affected areas and recommended contacting pest control for a service. Priority violation 7-38-020(a), citation issued

    - comments: observed dried food items such as dried pork, fish, and mollusks stored throughout the shopping ares in the facility in open containers. Instructed put all food items in containers that are smooth and easily cleanable and able to close and prevent contamination from customers.

    48. Warewashing facilities: installed, maintained & used; test strips
    - comments: observed the facility missing three sink stoppers for the three compartment sink. Instructed to provide adequate sink stoppers and maintain.

    49. Non-food/food contact surfaces clean
    - comments: observed build up of scales on the splash guard and on top of the three compartment sink. Also, the display cold holding units in the shopping with build up of food debris on the inside of the equipment. Instructed to clean and maintain saidequiment.

    - comments: observed a leak at the pipe under the washing compartment of the three compartment sink. Water profusely leaking into a bucket. Instructed to repair and maintain.

    - comments: observed outside garbage lids completely missing on both receptacles. Instructed to provide and maintain dumpster lids, to prevent rodent harborage. Priority foundation 7-38-020(b) citation issued.

    - comments: observed the floors thoughout the facility not maintain, especially in the walk-in cooler near the meat/seafood prep area which had fresh and old animal blood. Also, must elevate all items off the floors throughout the facility. Instructed to clean and maintain.

    - comments: observed employee utilizing gloves to handle raw steak and utilize same gloves, without handwashing, to handle ready-to-eat food items such as diced ham and sliced cheese. Instructed to discard mishandled items and wash hands... See More and replace gloves in-between tasks, such as switching from handling raw food items to ready-to-eat food items. Person-in-charge discarded mishandled food items and discussed proper handwashing procedures with employee. Priority violation, 7-38-010 citation issued.

    - comments: observed raw food stored over ready-to-eat foods in several areas throughout the kitchen area: raw shell eggs stored over ready-to-eat corned hash in the cook line area, raw beef stored over ready-to-eat potatoes in the serving line area, and raw eggs stored over ready-to-eat ham in the walk-in cooler. Instructed to relocate raw foods below ready-to-eat foods to prevent cross-contamination. Person-in-charge relocated raw eggs and raw beef below ready-to-eat foods on-site. Priority violation. 7-38-005 citation issued.

    22. Proper cold holding temperatures
    - comments: observed several food items stored in large walk-in cooler above 41. 0f: refried beans at 46. 8f, tuna at 46. 9f, pasta at 48. 7f, soup at 47. 8f. Instructed to discard and denature all out-of-temperature food items. Person-in-charge discarded and denatured all out-of-temperature food items on-site, approximately 50 lbs with $200 of product. Priority violation. 7-38-005 citation issued.

    25. Consumer advisory provided for raw/undercooked food
    - comments: missing disclosure for consumer advisory on the menu. Instructed to disclose on the menu which foods (ex, eggs, steak, hamburgers ) can be ordered under cooked/cooked to order with an asterisk next to those specific items; and then by asterisking them to a footnote that states these items are served raw or undercooked or contain (or may contain) raw or undercooked ingredients in addition to the reminder statement such as 'consuming raw or undercooked meats, poultry, seafood, shellfish, or eggs may increase your risk of foodborne illness, especially if you have certain medical conditions' priority foundation. 7-38-005.

    33. Proper cooling methods used; adequate equipment for temperature control
    - comments: observed large/rear walk-in cooler insufficient temperature to cold-hold tcs food items with internal ambient air temperature of 48. 9f. Person-in-charge discarded all tcs food items from cooler and ceased use of cooler to store tcs food items. Person in charge adjusted temperature of walk in cooler and maintain proper temperature of 36. 0f by end on inspection priority violation. 7-38-005 citation issued.

    - comments: observed cooked sausage and cooked bacon stored in direct contact with clean linens prior to service. Instructed to remove food items from linens and cease use of linens in contact with food and maintain.

    It is a new business and has nice food, drinks, and atmosphere, but about 24 hours later (edited timing after re-evaluating), my wife and I are vomiting and have diarrhea. UPDATE: We strongly suspect that it was Vibrio from the oysters. The vomiting and diarrhea lasted for a day and the nausea for another.
