On Friday 12.4 I had a sandwich at Subway in the evening and 2 hours later I had vomiting, diarrhea, stomach cramps and fever.

Ate a 6 inch Subway sandwich with ham, American cheese, lettuce, onions, mayo, and mustard along with a water bottle. This was approximately 12:30pm PST on 4/20/24. Ate nothing else today and the symptoms hit like a bus 3-4 hours later. the floor and picked it right back off the floor 😒 and place it on the counter top to be pick-up. I guess he didn't notice me looking at him, now if that's my order, i want a new order, not the one that hit the ground. Anyway, got home 🏡 ate my sub 2 to 4 hours later, scratch throat, feeling nausea ,stomach pain, fever, chills, headache then diarrhea, didn't no what the hail was going on, it's Apr 20th, 2024 and i'm still sick. - comments: observed clean linens in direct contact with food, used to cover dough in the walk-in cooler rack. Instructed to remove linens and only use linens in contact with food to line a food container for service.

41. Wiping cloths: properly used & stored
- comments: observed wiping cloths left out of sanitizer solution while wet at bar 3-compartment sink. Found wiping cloths stored in 3-compartment sink basin submerged in water with no sanitizer. Instructed to maintain wiping cloths used for wiping equipment surfaces in water with adequate sanitizer.

51. Plumbing installed; proper backflow devices
- comments: observed small leak at kitchen area handwashing sink drain plumbing. Instructed to repair handwashing sink drain plumbing and maintain plumbing fixtures in good repair.

55. Physical facilities installed, maintained & clean
- comments: observed sliding door to utility sink/water heater closet to be in poor repair and stuck. Instructed to repair or replace utility closet door and maintain facility and doors in good repair.
